MINOR WAIVER, RELEASE OF LIABILITY & PHOTO/VIDEO CONSENT

By submitting this form, I, the parent or legal guardian of the minor participant named below, give permission for the minor to participate in activities offered by Puppy Love Pilates, Yoga, and Beyond, LLC, including yoga, Pilates, puppy playtime, and interaction with animals.

I understand that participation involves inherent risks, including physical activity and interaction with puppies and other animals. These risks may include, but are not limited to, falls, strains, scratches, bites, allergic reactions, and other injuries. I knowingly assume these risks on behalf of the minor participant.

To the fullest extent permitted by law, I release and hold harmless Puppy Love Pilates, Yoga, and Beyond, LLC, its owners, employees, instructors, contractors, volunteers, participating breeders/rescues, and affiliated parties from claims arising from the minor’s participation, except to the extent such a release is prohibited by law.

I authorize photographs and/or video of the minor participant to be taken during the event and used by Puppy Love Pilates, Yoga, and Beyond, LLC for promotional, advertising, website, and social-media purposes.

I certify that I am the minor participant’s parent or legal guardian and have the legal authority to provide this consent on the minor’s behalf.
